FFAB - HEALTH REQUIREMENTS AND SERVICES: IMMUNIZATIONS

ADMITTANCE TO TSBVI

Each TSBVI student shall be fully immunized against diphtheria, rubeola (measles), rubella, mumps, tetanus, Haemophilus influenzae type B, and poliomyelitis. A child or student must meet all the immunization requirements specific to an age group upon first entering the age group. [See FFAB(EXHIBIT)]

The Board shall comply with any modifications or deletions in this requirement that may be made by the Texas Board of Health.

EXCLUSIONS

Immunization is not a prerequisite for admission to the School if the person applying for admission submits one of the following:

MEDICAL REASONS

  1. An affidavit or certificate signed by a physician who is registered and licensed to practice medicine within the United States, stating that in the physician's opinion the immunization required would be injurious to the health and well-being of the applicant or any member of her/his family or household. Unless a lifelong condition is specified, the affidavit or certificate is valid only one year from the date signed by the physician and must be renewed every year for the exclusion to remain in effect.

RELIGIOUS CONFLICT

  1. An affidavit signed by the applicant or, if a minor, by her/his parent or legal guardian stating that the immunization conflicts with the tenets and practices of a recognized church, religious denomination, or recognized religious organization of which the applicant is an adherent or member; provided, however, that this exemption does not apply in times of emergency or outbreak declared by the commissioner of health or local health authority.

PROVISIONAL ADMITTANCE

A student may be provisionally admitted to the School if the student has begun the required immunizations and continues to receive the necessary immunizations as rapidly as is medically feasible. The school must ensure that the required immunizations are received on schedule.

If a student transfers from one school to another, a grace period of no more than 30 days may be allowed at the new school while awaiting the transfer of the immunization record, during which time the student may be provisionally enrolled.

CONSENT TO IMMUNIZATIONS

The Director of School and Student Services or designee of the school may consent to the immunization of a minor if the School has received written authorization to consent from the parent or other person having the power to consent.

ACCEPTABLE DOCUMENTS OF IMMUNIZATION

Any validated document of immunization presented by a student is acceptable, provided it shows the day, the month, and the year when each immunization was received. The day, the month, and the year of vaccine or disease must be recorded in all new school records initiated, and in existing records updated, after September 1, 1991.

All new entries in immunization files must be based upon a certificate of immunization that has been validated by, or for, a physician or public health personnel. All immunization records created on or after September 1, 1995 must be a complete and accurate record of all validated immunization histories available for each child. These records must include all doses administered to a child, including all doses that exceed the minimum mandated requirements. The validated record should be returned to the student, or the student's parent or guardian, after the immunization history has been entered in school records.

The School shall maintain an individual immunization record during the period of attendance for each student admitted.

The records shall be open for inspection at all reasonable times by the Texas Education Agency, or by representatives of local health departments, or the Texas Department of Health.

The Texas Department of Health and local health departments will provide record-keeping cards free of charge for maintaining school immunization files, or commercially or locally produced record cards may be used.

VERIFICATION OF IMMUNITY

Serologic confirmation of immunity to measles, rubella, mumps, hepatitis A, or hepatitis B are acceptable in lieu of vaccine against the serologically confirmed disease. If a child is unable to submit serological proof of immunity or serologic evidence of infection, then measles, rubella, mumps, hepatitis A, or hepatitis B vaccine is required. Evidence of measles, rubella, mumps, hepatitis A, or hepatitis B illnesses must consist of a laboratory report indicating confirmation of immunity or confirmation of infection. The school shall accurately record the results of any serologic tests supplied as immunity, and the original should be returned to the student or to the student's parent or guardian, as appropriate.

All histories of varicella illness must be supported by a written statement from a physician or the student's parent or guardian. School nurses may also write this statement to document cases of chicken pox that they observe. The school shall accurately record the existence of any statements attesting to previous varicella illness or the results of any serologic tests supplied as proof of immunity. The original should be returned to the student or the student's parent or

guardian. If a student is unable to submit such a statement or varicella evidence, varicella vaccine is required.

ANNUAL REPORT OF IMMUNIZATION STATUS

The Texas Education Agency and the Texas Department of Health shall develop the form for a required annual report of the immunization status of students, and such annual report shall be submitted by the School at such time and in such manner as is indicated in the instructions printed on such form.

TRANSFER OF RECORDS

The School shall cooperate with other districts in transferring students' immunization records to other schools.

Specific approval from students, parents, or guardians is not required prior to making such record transfers.

When a student transfers from one school or district, to another, a copy of the immunization record, any laboratory reports or statement of prior illness accepted in lieu of vaccination, any affidavits or statements of medical contraindications, any statements for religious exemption, and laboratory reports of immunity should be sent within thirty days to the receiving school. A record received from school officials of another district, or state, may be considered a validated record.
